Sentences should be short and language should be simple. Short sentences help in quickly reviewing the document, which saves time of the higher management. The format can be alliteratively evolved from the feedback but it should be as terse as possible. Point wise summary points are more preferred than long paragraphs. It must be as factual as possible and it doesn’t have space for suggestions or reasons for failure, thus try to avoid them.
